With the federal Ready, Set, PrEP program gone and Gilead’s free Truvada PAP ended, the landscape of free PrEP has shifted dramatically. But more pathways to $0 PrEP exist than ever — if you know where to look. Here’s a ranked guide to every government-funded, manufacturer, and telehealth program that can get you PrEP for free.
1. Telehealth Platforms (Fastest & Easiest)
If you want PrEP today with the least friction, start here. These platforms handle everything — consultation, labs, prescriptions, and delivery — from your phone.

2. Manufacturer Patient Assistance Programs
Drug manufacturers offer the most comprehensive free medication programs, especially for uninsured patients.
Gilead Advancing Access
Uninsured: Free medication via MAP for patients with income ≤500% FPL (~$75,300/yr). No SSN required. Undocumented residents eligible. Medication ships via FedEx overnight.
Insured: Copay card covers up to $7,200/yr (Descovy) or $8,000/yr (Yeztugo). Yeztugo patients also get $100/visit for injection administration.

3. Insurance Coverage (ACA Mandate)
If you have health insurance — employer-sponsored, ACA marketplace, Medicaid, or Medicare — PrEP is almost certainly covered at $0 cost-sharing. This includes medications, quarterly lab work, and clinical visits.
The ACA requires all non-grandfathered private insurance plans to cover USPSTF Grade A preventive services without cost-sharing. PrEP has carried this designation since 2019. If your insurer charges you anything for PrEP-related services, they may be violating federal law.
What If My Insurance Denies Coverage?
File an internal appeal with your insurer citing the USPSTF Grade A recommendation (PrEP for persons at high risk of HIV acquisition, Grade A, 2019). If denied again, file a complaint with your state insurance commissioner. While your appeal is pending, use a telehealth platform like MISTR to maintain uninterrupted PrEP access.
4. State PrEP Drug Assistance Programs (DAPs)
About 12 states operate dedicated PrEP DAPs that cover medication, labs, and sometimes clinical visits for eligible residents. These programs are especially valuable for uninsured or underinsured patients who don’t qualify for manufacturer PAPs.

5. Community Health Centers (FQHCs)
Federally Qualified Health Centers serve patients regardless of ability to pay, on a sliding fee scale. Many FQHCs are 340B-eligible, meaning they can access medications at steep federal discounts and pass savings to patients. Over 1,400 FQHCs nationwide prescribe PrEP.

6. Nonprofit Copay Assistance
These foundations help insured patients cover out-of-pocket costs that remain after insurance:
Patient Advocate Foundation (PAF)
Up to $7,500/year. Income ≤400% FPL. Insured patients only. SSN required. PAF and PAN Foundation announced a merger in March 2026 — a unified “TotalAssist” program launches July 1, 2026.
Good Days Foundation
Up to $7,500/year. Income ≤500% FPL. Primarily for Medicare/military beneficiaries. SSN required. Note: PrEP fund was closed as of April 2026 — sign up for alerts when new grant money is available.
Programs That Have Ended
