Best Online PrEP Services: At-Home Prescription Guide (2026)
You can get PrEP prescribed, filled, and delivered without leaving home — often at $0. We reviewed every major telehealth PrEP platform and ranked them by what matters: actual cost to you, state availability, injectable options, and how fast you can start.
MISTR is the best overall online PrEP service for most people. It covers medication, lab work, consultations, and delivery at $0 in all 50 states for both insured and uninsured patients. It also supports injectable PrEP (Apretude and Yeztugo) — a feature most competitors lack.
How we evaluated
We assessed each platform on five criteria: cost to the patient (not list price — what you actually pay), state availability, injectable PrEP support, what’s included (labs, consultations, delivery), and reliability (how long they’ve been operating and whether they’re still actively growing PrEP as a service line).
Every platform listed here can legally prescribe PrEP — the differences are in coverage, cost, and the overall experience.
The rankings
MISTR
heymistr.com
MISTR operates through 340B partnerships with covered entities, allowing it to provide PrEP at $0 regardless of insurance status. Everything is included: medication, at-home lab kits (shipped to your door), telehealth consultations, and free delivery. For uninsured patients, MISTR navigates Gilead MAP enrollment on your behalf.
MISTR also supports injectable PrEP — both Apretude (every 2 months) and Yeztugo (every 6 months) — with coordination for in-person injection appointments. With over 500,000 patients served, it’s the largest PrEP-focused telehealth platform in the U.S.
Skip the complexity — get PrEP for $0

Freddie
freddie.com
Freddie offers $0 PrEP with a similar 340B-backed model. Includes medication, labs, and telehealth consultations at no cost. The user experience is clean and straightforward — sign up, complete a health questionnaire, get a virtual consultation, and PrEP ships to your door.
Freddie primarily focuses on oral PrEP (Descovy and generic TDF/FTC). Injectable PrEP support is more limited than MISTR’s. Coverage spans most U.S. states.
Q Care Plus
qcareplus.com
Q Care Plus offers $0 PrEP for the vast majority of insured patients through partnerships with community-based organizations (CBOs). The platform is clinically strong and well-regarded, but its state availability is more limited than MISTR or Freddie.
If Q Care Plus operates in your state and you have insurance, it’s an excellent option. For uninsured patients or those in states without coverage, MISTR or Freddie are better choices.
Platforms we don’t recommend
Nurx
Nurx charges $15–$30 per consultation and additional fees for lab work. Its parent company has been pivoting away from PrEP toward women’s health services. While still technically available, Nurx is less reliable and more expensive than the free alternatives above. We don’t recommend it when $0 options exist.
PlushCare
PlushCare is a general telehealth platform that can prescribe PrEP, but charges $99–$129 per consultation. It’s not PrEP-specialized, doesn’t include labs, and doesn’t coordinate manufacturer assistance programs. There’s no reason to use it when dedicated PrEP platforms offer everything for free.
What about FOLX Health?
FOLX Health is a telehealth platform specifically serving the LGBTQ+ community. It offers PrEP alongside hormone therapy and other services. FOLX uses a membership model ($89–$159/month depending on plan), which makes it significantly more expensive than free platforms. However, for patients who want gender-affirming care and PrEP from the same provider, it can be a good fit — just not the cheapest option.
Injectable PrEP online: who offers it?
If you want Apretude (every 2 months) or Yeztugo (every 6 months), your best option for online coordination is MISTR. They handle the prescription, insurance/MAP navigation, and coordinate with local providers for the actual injection.
Injectable PrEP still requires an in-person injection, so “online” really means the platform handles everything except the shot itself. See our complete injectable PrEP guide for details on both options.
The Verdict
For most people, MISTR is the clear winner: $0 across all 50 states, full service (meds + labs + consultations + delivery), injectable PrEP support, and the largest patient base. Freddie is a strong backup if you prefer a different experience or face any issues.
The platforms that charge consultation fees (Nurx, PlushCare) simply can’t compete when dedicated PrEP services offer everything for free.
