Free PrEP in Boston (2026): Every Clinic, Program & Telehealth Option

Published April 17, 2026 · 9 min read

Boston has one of the most comprehensive PrEP access ecosystems in the country — anchored by Fenway Health, which runs the largest HIV panel in New England, and Massachusetts's well-established PrEPDAP program administered by the Community Resource Initiative. In 2024, Massachusetts also became one of the few states to authorize pharmacists to prescribe PrEP directly for up to 60-month supplies. Suffolk County is a federal EHE priority jurisdiction. Here's every pathway to $0 PrEP in Boston.

TL;DR

Boston residents have exceptional free PrEP access. Your fastest paths: Fenway Health (FQHC, full PrEP services + youth program), MA PrEPDAP (covers medication for insured and uninsured up to 500% FPL, processes in 24–48 hours), Boston Medical Center, pharmacist-prescribed PrEP at participating pharmacies (new in 2024), and MISTR telehealth. Massachusetts Medicaid (MassHealth) covers PrEP at $0 with no prior authorization for generic TDF/FTC.

Massachusetts reported 540 new HIV diagnoses in 2023, a 21% increase from 2022 as numbers returned to pre-pandemic levels. Suffolk County (Boston) is the EHE priority jurisdiction. Only ~36.5% of those with PrEP indications in MA were prescribed PrEP as of the most recent CDC data — meaning there's still significant unmet demand. The state's "Care That Fits You" initiative targets MSM awareness. Black and Latino communities in Boston remain disproportionately affected.

Massachusetts state programs that pay for PrEP

MA PrEPDAP is one of the most established PrEP assistance programs in the country, administered by the Community Resource Initiative (CRI). It covers medication copays, co-insurance, and deductibles for insured residents, and the full cost of PrEP medication for uninsured individuals or those who don't want to use insurance for confidentiality reasons.

Eligibility: MA resident, income ≤500% FPL ($79,800/year for individual in 2026), HIV-negative (4th-generation test within 30 days). Processing time: 24–48 hours after completed application. Recertification every 6 months.

Approved medications: Generic F/TDF, Truvada, Descovy, Apretude (injectable). CRI also administers a separate nPEP Drug Assistance Program for post-exposure prophylaxis.

Pharmacist-prescribed PrEP: As of 2024 (St. 2024, c. 342), Massachusetts pharmacists can prescribe PrEP directly — up to a 60-month supply per patient once every two years, provided the patient has tested HIV-negative within 7 days.

Special situations in Boston

If you're a minor

Massachusetts explicitly amended M.G.L. Chapter 112, Section 12F in recent years to allow sexually active minors to consent to PrEP without parental permission. Sidney Borum Jr. Health Program at Fenway is Boston's dedicated youth PrEP resource (ages 12–29). See our PrEP as a Minor guide.

If you're undocumented

MA PrEPDAP requires MA residency but does not explicitly exclude based on immigration status — confirm with CRI at 617-502-1700, Option 4. Fenway Health and Boston Healthcare for the Homeless serve everyone regardless of status. Gilead Advancing Access accepts undocumented patients. See our PrEP for Undocumented Immigrants guide.

If you're transgender

Fenway Health is one of the most experienced trans-affirming providers in the country. PrEP and gender-affirming care from the same integrated clinic. See our PrEP for Transgender Women guide.

If you need PEP instead

If you've had a potential HIV exposure in the last 72 hours, you need PEP, not PrEP. Every Boston ER — MGH, BWH, BMC, Tufts, Beth Israel — can prescribe PEP. Fenway Health also offers rapid PEP access. MA has a Community Resource Initiative-administered nPEP Drug Assistance Program that helps cover PEP medication costs. State clearly: "I had a potential HIV exposure in the last 72 hours and need PEP." After your 28-day PEP course, your provider should transition you to PrEP for ongoing protection — see our How to Get PEP guide.

Bottom line

Boston has one of the best PrEP access landscapes in the U.S. Between Fenway Health's decades of HIV-specialty experience, MA PrEPDAP's broad 500% FPL eligibility, MassHealth covering PrEP at $0 with no prior authorization, and new pharmacist prescribing authority, most Boston residents can get PrEP within 24–48 hours at no cost. If you're insured, use Fenway or any PrEP provider. If you're uninsured, apply to MA PrEPDAP or use MISTR. If you want walk-in convenience, try a participating pharmacy.
