Pennsylvania doesn't have a state PrEP-DAP, but Philadelphia has an unusually strong free PrEP ecosystem: the Philadelphia Department of Public Health operates PrEP clinics citywide regardless of insurance, PrEP2Me offers completely free TelePrEP state-funded for PA residents, and Mazzoni Center is one of the largest LGBTQ+ FQHCs in the Northeast. Philadelphia's HIV prevalence rate (1,157 per 100,000) is nearly four times the Pennsylvania state rate, and Black Philadelphians — 43% of the population — represent 64% of people living with HIV. The city has responded with an exceptionally dense PrEP provider network and — critically — a state-funded free TelePrEP program. Pennsylvania state programs that pay for PrEP

Pennsylvania does NOT have a state PrEP Drug Assistance Program. The Special Pharmaceutical Benefits Program (SPBP) only covers HIV treatment, not prevention. Its income threshold was actually lowered from 500% to 350% FPL in October 2025 due to budget pressure.

What Pennsylvania does offer: PrEP2Me. This is a PA DOH-funded, completely free TelePrEP program available to any PA resident. It's operated by Central Outreach Wellness Center in Pittsburgh but serves the whole state. Call 412-844-PREP or visit prep2me.com.

Special situations in Philadelphia

If you're a minor

Pennsylvania law allows minors of any age to consent to STI/HIV testing and treatment without parental permission. A federal court has interpreted this to include HIV prevention services — meaning PrEP. St. Christopher's Hospital is Philadelphia's go-to youth PrEP resource (ages 13–24). If you need PEP instead

If you've had a potential HIV exposure in the last 72 hours, you need PEP, not PrEP. Philadelphia ERs can prescribe PEP — Jefferson, Penn Medicine, Temple, and Einstein all have protocols. Philadelphia FIGHT also offers rapid PEP access through its telehealth line (215-545-PREP). State clearly: "I had a potential HIV exposure in the last 72 hours and need PEP."
