What's Happening
PAF and PAN, which together have helped millions of patients afford medications through copay assistance, are combining into a single organization called TotalAssist. The official launch date is July 1, 2026.
How This Affects PrEP Patients
Both PAF and PAN have operated disease-specific funds that help patients with out-of-pocket costs for medications, including HIV prevention and treatment. If you've received copay assistance through either foundation, here's what to know:
- Existing grants should continue: If you have an active grant from PAF or PAN, it's expected to carry over to TotalAssist.
- Application process may change: The merged organization will likely have a unified application. Watch for communications from PAF or PAN about how to transition.
- Fund availability: Foundation copay funds open and close based on available funding. The merger could affect timing and availability. Apply early.
Remember: Foundation copay assistance is a supplement, not a primary pathway. If you have insurance, PrEP should be $0 under the ACA mandate. If you're uninsured, manufacturer programs (Gilead MAP, ViiV) and telehealth (MISTR) are your primary options.
Don't rely solely on foundations
MISTR provides complete PrEP at $0 — no copay cards, no foundation applications, no annual renewals to manage.

When You Need Copay Assistance for PrEP
You generally only need third-party copay assistance if:
- You have insurance that's charging you for PrEP (which may violate the ACA — see our insurance guide)
- You're on a grandfathered plan not subject to ACA preventive services mandates
- You're on Medicare Part D and need help with costs before reaching the OOP cap
- Manufacturer copay cards don't cover your specific situation
Your PrEP Cost Safety Net (In Order)
| Layer | Program | What It Covers |
|---|---|---|
| 1st | ACA insurance mandate | $0 PrEP — medication, labs, visits |
| 2nd | Manufacturer copay card (Gilead, ViiV) | Up to $7,200–8,000/year in copays |
| 3rd | TotalAssist / foundation funds | Additional copay/coinsurance help |
| 4th | State PrEP DAP | Medication and/or lab costs (12 states) |
| Backup | MISTR (telehealth) | Everything at $0, any insurance status |
The simplest path is still free
Skip the paperwork. MISTR provides free PrEP end-to-end — consultation, labs, medication, delivery. No copay cards needed.
