Best Online PrEP Providers Compared

An honest comparison of every major telehealth PrEP platform — cost, coverage, state availability, and what's actually free vs. what isn't.

Telehealth has become one of the fastest-growing ways to access PrEP. An estimated 20% of all US PrEP users (~110,000 people) received PrEP through telehealth platforms in 2024. These services handle the entire process remotely — consultation, lab work, prescription, and medication delivery.

But not all platforms are equal. Some are genuinely free. Others charge consultation fees, lab fees, or have limited state availability. Here's how they compare.

How we rank: We prioritize cost to you first, then coverage scope (states, labs, injectable options), then user experience. We earn referral commissions from some platforms, but this never changes our ranking — the cheapest, most comprehensive option wins. Full disclosure →

FOLX Health

FOLX Health is a queer and trans-focused telehealth platform offering PrEP alongside hormone therapy and other LGBTQ+-specific healthcare. It's membership-based, which adds cost, but the care is affirming and tailored. A good fit if you want PrEP from a provider that specializes in queer and trans health — but not the most cost-effective choice for PrEP alone.

The Bottom Line

If your primary goal is free PrEP with the least hassle, MISTR is the clear winner — $0 everything, all 50 states, at-home labs, and the broadest service scope. Q Care Plus and Freddie are strong alternatives where available. Nurx and PlushCare charge fees that are unnecessary given the free options available.

The key distinction is the 340B model. Platforms that partner with 340B-eligible covered entities (MISTR, Q Care Plus) can genuinely provide $0 PrEP because the economics work through insurance reimbursement margins — not by charging you. Platforms without 340B partnerships need to charge fees to cover their costs.

Don't forget manufacturer programs. Regardless of which telehealth platform you use, Gilead Advancing Access and ViiVConnect provide free medication or copay cards. These can be combined with any provider. See all programs →
