Free PrEP in West Virginia

West Virginia has no PrEP DAP, no pharmacist prescribing, and is not an EHE priority state — despite experiencing some of the nation's worst injection-drug-use-related HIV outbreaks. 23.3% of PLWH acquired HIV through IDU (3rd highest nationally). Medicaid expansion covers PrEP at $0, but only ~10 syringe service programs remain and legislation threatens to ban them entirely.

Insurance Coverage

Under the ACA, all private insurance plans must cover PrEP at $0 cost-sharing. Medicaid covers nearly half a million West Virginians.

IDU Prevention

PrEP reduces HIV from injection drug use by ~74% but is severely underutilized in WV. SB 334 (2021) restricted syringe services, and legislation to ban SSPs entirely is advancing.

HIV Laws

No HIV-specific criminal statutes, but general criminal laws can be applied.

Rural Access

WV is ~50% rural with only 3 metro areas. 34 FQHC organizations with 460+ locations provide the critical safety net. Broadband gaps limit telehealth adoption in some areas.
